O-Desmethyltramadol or O-DSMT for short (also called desmetramadol, Omnitram) is an opioid substance of the phenylpropylamine class, a highly active metabolite of tramadol with sedative and analgesic properties.

The atypicity of O-Desmethyltramadol in the form of its stereoisomers is a reflection of each other in three protections. The difference between O-DSMT and tramadol is in the absence of a methyl group in R3.
O-DSMT is a stronger agonist of the μ-opioid receptor than tramadol, and also affects the δ- and κ-opioid receptors.
O-DSMT is a norepinephrine reuptake inhibitor, but is not active as a serotonin reuptake inhibitor, which makes it useful in the treatment of various types of pain.
This drug affects the m-opioid receptors, activating them they mimic endorphins, which leads to euphoria and anxiolytic effect, anxiety suppression and relaxation.
The action of O-DSMT is less active compared to morphine.
